Why do I need these forms?
Once you have mailed your FAFSA or completed the FAFSA on the Web AND provided that you have listed Sierra College Title IV Code 001290 on it, you will receive a Missing Information letter from us.
The Missing Information letter may request the following documents to be completed by you. These documents are not required of ALL of our student population, it depends on your situation, please read your Missing Information letter (MIL) carefully to determine which documents are required for you to complete your file and receive financial aid.

Download the fee waiver application, complete and print the form. Submit the completed form to The Financial Aid Office. Eligibility is determined at that time. The BOGW is available to California residents who meet income eligibility requirements. The BOGW should be completed before registration. The program waives the $20 per unit enrollment fee, the $2 PASS telephone registration fee, the $15 health fee, and also reduces the semester parking fee from $40 to $20. The BOGW is effective for the whole academic year (Fall, Spring and Summer) and there is no minimum unit requirement. The BOGW application is based on income earned in the prior calendar year. Complete this form only if you have failed completing all your units and/or maintaining cumulative GPA of 2.0 or higher and was terminated from financial aid. You will also need to attach a current and comprehensive Student Educational Plan (SEP) completed by a Sierra College Counselor showing remaining classes and time required for program completion.

If you have a Cal Grant A, B, or C, use the G-10 Form (.pdf) to report any changes in your name, social security number, address, school, or to request a leave of absence.
After you complete and sign the form, mail it to the Financial
Aid Office, Sierra College, 5000 Rocklin Rd, Rocklin, CA 95677;
bring it to the office, which is located in the Winstead Center
on the Rocklin Campus; or fax it to (916) 789-2905. Completed forms
can also be dropped off at the Nevada County Campus.
